Guidelines for Grant Applicants

• Applicants must be exempt under Section 501 (c) 3 of the Internal Revenue Code.
• Applicants must be Arizona based and commit to spend any grant proceeds within Arizona.
• Applications may be handwritten or typed in the allowable space on the following form or in an identical format.
• Once the application packet is completed, contact the Fred W. Stang Foundation via email at Info@FredWStang.org. A Board member will be assigned as your contact. A mailing address will be given by your contact.
• Any questions prior to receiving a contact, may be sent to Info@FredWStang.org. The FAQ (frequently asked questions) may be a helpful resource page.
• The Board will review and grant requests at the end of each year. Grant requests must be post marked by October 31st to be considered.

Provide the year your organization was established and a description of services that your agency provides.

Provide a list of your Board of Directors, the length of time on Board, and their current occupations.
